Neuroanesthesia and Cerebrospinal Protection

Hiroyuki Uchino , Kazuo Ushijima
e altri

editore: Springer Verlag Japan

pagine: 725

This book is a knowledge-based text that covers the anatomy, neurophysiology, and neuropharmacology needed to carry out the necessary management and cerebrospinal protection during neuroanesthesia. Especially to make the subject of neuroanesthesia more approachable, we introduce surgical measures utilized in neurosurgery, cardiovascular surgery, neuromodulation, and related procedures. The main purpose of neuroanesthesia/neurocritical care is to deliver cerebrospinal protection in neurosurgical and cardiovascular surgeries. How should we select the best anesthetic management to prevent neurological complications for patients who, for instance, undergo different types of surgery for subarachnoid hemorrhage, stroke, head trauma, carotid endarterectomy, or cardiovascular surgery under cardiopulmonary bypass? These pathological conditions include the risk of transient cerebrospinal ischemia, and if our management is not precise, it may induce serious neurological sequelae. Although there is an urgent need to establish the treatment and elucidate the molecular mechanisms of cerebrospinal injury, many components are intertwined, making this a challenge we have not completely solved yet. This volume makes the management of neuroanesthesia more accessible not only for primary residents but also for specialists, providing a valuable resource on the current perspectives of neuroanesthesia.

Gasless Single-Port Robosurgeon Surgery in Urology

Kazunori Kihara

editore: Springer Verlag Japan

pagine: 184

This book focuses on a novel series of minimally invasive surgeries performed without gas insufflation and through a single port using the RoboSurgeon system. The concept of RoboSurgeon surgery involves a degree of "robotization" of the operator and participants. The operator, wearing a head-mounted display, uses robot-like handheld instruments. The endoscope is controlled by the movement of the operator's head using a gyro-sensor system. All participants wearing head-mounted displays can see the same images as the operator does. Although this surgery is mainly intended for urological cancers, it may also be applicable for various operations concerning other urological diseases, as well as for the treatment of diseases in different medical fields. The system has the potential to become the new standard in robotic surgery. This solution may be a boon to many patients around the world, especially in the aging societies many countries are now facing.

Writing a Biomedical Research Paper

Brian Budgell

editore: Springer Verlag Japan

pagine: 74

All of us in biomedicine understand the urgency of getting experimental results into print as quickly as possible. Yet this critical step in the cascade from research conception to publication receives almost no attention in our formal training. It is as if we have been put to sea without a compass. Our collective failure to achieve widespread literacy in our own language - Biomedical Language - seriously impedes the important process of d- seminating new biomedical knowledge and thereby improving the human condition. It is also a significant personal concern for researchers and clinicians in the highly competitive, publish-or-perish environment of c- temporary academia. Of course, if we are clever or lucky enough to come up with that Nobel Prize-winning discovery, great science will carry the day and we are likely to get published even if our writing is fairly horrid. But most of us who publish are "bread-and-butter" scientists. We compete for space in journals which may only accept 10% or 20% of the submissions that they receive each year. For us, convincing, engaging writing will make the difference between being published or rejected, or at least it will make the difference between being published on ? rst submission or having to go through a number of revisions (or journals). None of this is to propose that good writing can make a silk purse out of a sow's ear. Scienti? c content is the sine qua non of biomedical writing.

Operation Atlas of Hepato-pancreato-biliary Surgery

Collected Case Studies

Hidenori Miyake , Seiki Tashiro

editore: Springer Verlag Japan

pagine: 342

This work bridges a gap that exists in the field of hepato-pancreato-biliary surgery: the scarcity of surgical atlases dealing with the individual peculiarities that change from patient to patient and which are of critical importance in determining the outcome of an operation. Previous atlases dealt with the subject in broad strokes, for example advocating right hepatic lobectomy for liver cancer or pancreatoduodenectomy for cancer of the pancreatic head. That approach takes little or no account of the needs of the individual, which can vary greatly, even in patients with similar conditions or whose cancer has spread to the same extent. The editors have produced an experience-based atlas that illustrates just how effective surgery can be when the finer details of previous cases are considered. Even in cases of advanced cancer, which account for 80 per cent of the studies in this book, remarkable results are possible when the appropriate techniques are fully utilized.

HCV/Oxidative Stress and Liver Disease

Kiwamu Okita

editore: Springer Verlag Japan

pagine: 120

Since the discovery of superoxide dismutase more than three decades ago, there has been rapid growth in the knowledge of oxidative stress and disease. This volume containing the proceedings of the 13th Yamaguchi Symposium on Liver Disease includes discussion of the direct cellular effects of hepatitis C virus (HCV) proteins on hepatocytes and reviews evidence that oxidative stress caused primarily by the HCV core protein plays a key role in disease pathogenesis. Also included are chapters on new aspects of oxidative stress and liver disease such as carbon monoxide as a regulator of liver microcirculation, hepatic iron accumulation and the incidence of hepatocellular carcinoma, and oxidative stress in the absence of inflammation in hepatocarcinogenesis. This collection of papers from the Yamaguchi Symposium creates a valuable reference resource for physicians and hepatologists.

Arthroplasty 2000

Recent Advances in Total Joint Replacement

N. Matsui , Y. Taneda
e altri

editore: Springer Verlag Japan

pagine: 268

Joint replacement surgery has seen remarkable progress and development in recent years both in prostheses and in surgical technique. A prime concern has been improved durability, for which a major factor is reduction of the polyethylene wear that leads to osteolysis. This book presents an update on the means by which the problems of wear and loosening are being addressed in total hip arthroplasty (THA) and total knee arthroplasty (TKA). Included are chapters on new surgical techniques for difficult cases, nonpolyethylene interfaces for THA, custom hip prostheses, and computer-assisted surgery. Arthroplasty 2000 also takes up current controversial issues such as posterior cruciate substitution versus retention in TKA. With contributions by eminent specialists in total joint replacement in Asia, Europe, and North America, this volume is a valuable reference for all orthopedic surgeons.

The Middle Ear

The Role of Ventilation in Disease and Surgery

Hideaki Takahashi

editore: Springer Verlag Japan

pagine: 116

The middle ear is an organ that, like the lung, must maintain an aerated cavity within it to carry out its function. When hypoventilation occurs in the lung it threatens the maintenance of life, when it occurs in the middle ear it causes loss of hearing. For the middle ear to fulfill its vital sound-conduction function, its pressure must be maintained at approximately atmospheric level. The eustachian tube has long been known to play a key role in pressure regulation, but later research has revealed that transmucosal gas exchange is as important as the function of the eustachian tube, and that the ventilatory physiology of the middle ear is maintained by these two mechanisms in tandem. The findings presented in this volume answer many questions about ventilation and pressure regulation of the middle ear, and should be valuable to clinical practitioners in choosing appropriate modes of treatment for pathological conditions such as cholesteatoma and otitis media.
