Complications in Knee and Shoulder Surgery

Jeffrey Halbrecht , Robert J. Meislin

editore: Springer London Ltd

pagine: 294

"To Err is Human..." Alexander Pope Any surgical intervention has potential risks and complications. This book is designed as a reference tool and a roadmap for the sports medicine orthopedist to minimize the complications in knee and shoulder surgery. Some complications are avoidable; some are not. By understanding the functional anatomy at risk, proper surgical setup, proper equipment, and proper patient selection, a preventative approach to complications can be instituted. The authors of these chapters represent leaders in orthopedic sports medicine and have provided novel methods for dealing with intraoperative complications and si- ations that can occur in the midst of a "routine" arthroscopic reconstruction. In d- cussing these issues, we hope to raise the surgeon's awareness of potential complications and to provide wisdom and advice to properly avoid and treat these problems should they arise. How one deals with complications truly tests the mettle of the surgeon. Keeping one's "cool" in the midst of an intraoperative complication will be facilitated by h- ing an algorithmic approach. We hope that this book will improve your outcome should these problems occur. Some complications are unavoidable. Arthro brosis or stiffness may take place in the best of hands. Infection may surface postoperatively in the most sterile OR. We provide you with the background to confront these challenges. A list of ten "Pearls" are offered at the beginning of most chapters as a quick reference card.

The Infected Implant

Heino Kienapfel , Klaus-Dieter Kuhn

editore: Springer-Verlag Berlin and Heidelberg GmbH & Co. K

pagine: 139

Musculosketal infections are potentially devastating for the patient, his family and the health care services, as they may lead to functional impairment, long lasting disability, or even permanent handicap. This problem may also have socioeconomic implications. Infection commonly results in prolonged use of antibiotics, multiple operations, complications, extended rehabilitation and long lasting follow-up visits with additional costs for the health care provider. The 2nd International Arthroplasty Symposium "The Infected Implant" brought together international experts and will concentrate on infections: development, microbiology, diagnostics, statistics and therapy of the infected implant. Concerning microbiological diagnostics, a major challenge is the differentiation between septic and aseptic prosthesis loosening. Biofilm formation at low grade infections will therefore be one of our main topics at the event in 2008. Furthermore, the issue of antibiotic resistances, which is always a hot topic, will be discussed, based on new clinical and epidemiological data. These days, a number of options for the therapy of the infected implant are available. Lectures about the rational use of antibiotic medication and theapplication of antibiotic containing spacers will represent key topics of this meeting. The question whether revision surgery should be cemented or not will be another subject of the panel discussion, where clinicians of different countries worldwide will present their revision cases. A third part of the symposium will cover consequences and economical aspects of infected implants. The speakers will focus on the different European Arthroplasty Registers, but the issue of reimbursement practices within the EU will also be discussed.

Hemiarthroplasty for a Comminuted Fracture of the Proximal Humerus

Darren J. Friedman , Jon J. P. Warner
e altri

editore: American academy of orthopaedic surgeons

This title focuses on such areas as: shoulder & upper extremity and trauma. Hemiarthroplasty is a common surgical treatment for certain proximal humerus fractures, including 4-part fracture in older individuals, 3-part fracture associated with osteopenia, head-split fracture, impaction fracture greater than 40 per cent of the articular surface, and anatomic neck fracture. Anatomic reconstruction of proximal humeral anatomy in these fractures is challenging. To enhance tuberosity healing and recreate normal joint mechanics, the surgeon must properly restore the humeral head height, size, offset, and version. In this video, we detail our technique of using hemiarthroplasty for the treatment of proximal humerus fractures. Height is determined by referencing off the pectoralis major insertion, and tuberosities are positioned at roughly 90 degrees to one another. Tuberosity reconstruction and healing is critical to successful functional outcome.
