It is only recently that the use of the endoscope as the sole visualizing tool has been introduced in transsphenoidal pituitary surgery with its favorable related implications and minimal operative trauma. Of course, microscopic and endoscopic anatomy are basically the same, but the optical distorsion of endoscopic images is quite substantial compared to microscopic depictions. An endoscope lens produces images with maximal magnification at its center and severe contraction at its periphery. Nearer images are disproportionally enlarged and remote images are falsely miniaturized. This optical illusion may disorientate a surgeon who is not familiar with this peculiar condition at the skull base. This atlas acts as a guide through the endoscopic anatomy and gives detailed descriptions of the preoperative management and the surgical procedures.

The middle ear is an organ that, like the lung, must maintain an aerated cavity within it to carry out its function. When hypoventilation occurs in the lung it threatens the maintenance of life, when it occurs in the middle ear it causes loss of hearing. For the middle ear to fulfill its vital sound-conduction function, its pressure must be maintained at approximately atmospheric level. The eustachian tube has long been known to play a key role in pressure regulation, but later research has revealed that transmucosal gas exchange is as important as the function of the eustachian tube, and that the ventilatory physiology of the middle ear is maintained by these two mechanisms in tandem. The findings presented in this volume answer many questions about ventilation and pressure regulation of the middle ear, and should be valuable to clinical practitioners in choosing appropriate modes of treatment for pathological conditions such as cholesteatoma and otitis media.

Il maestro di canto può comunicare, meglio e con maggiore efficacia, i suoi insegnamenti attraverso l’uso di strumenti che traducono in immagini il comportamento vocale dell’allievo che da parte sua ne ricava tutti i vantaggi. Il foniatra dispone di sofisticate apparecchiature, videoendoscopiche e di registrazione del fenomeno sonoro e delle strutture anatomiche dell’organo vocale, traendone informazioni obbiettive più sicure che può trasmettere al maestro di canto per chiarire eventuali dubbi sulla classificazione vocale e sulla salute dell’apparato fonatorio dell’allievo. Nella prima parte, com’è prassi, ho riportato quanto fin qui si considera acquisito circa gli aspetti anatomo-funzionali e fisici della voce cantata e sui criteri e metodi di classificazione, argomento a cui abbiamo portato un contributo che riteniamo importante per averne sperimentato la validità che è frutto di ricerche effettuate presso il reparto foniatrico della prima clinica ORL dell’Università di Roma “La Sapienza”. Riguardo alla semeiotica, ho trattato più diffusamente quelle metodiche d’indagine, soggettive ed oggettive strumentali, che possono fornire le informazioni più importanti ai fini di una corretta diagnosi clinica. Sulla patologia della voce cantata, per mia esperienza e con il conforto di studiosi che ne riferiscono nella letteratura più recente, sostengo che esistono, in percentuale importante ai fini statistici, forme cliniche disfunzionali pure accanto ad altre miste ed a quelle organiche che si osservano più frequentemente nei cantanti. Ho trattato diffusamente l’etiologia delle disodie disfunzionali perchè ritengo essenziale che si capisca quale, nel singolo caso, sia stata la causa della fonopatia, per poterla poi eliminare o correggere. Infine ho voluto attirare l’attenzione dei cantanti professionisti su quelle norme d’igiene indispensabili a mantenere sano ed efficiente lo strumento vocale che nella pratica clinica ho spesso riscontrato disattese dagli interessati. La pedagogia del canto artistico, opera del maestro Valerio Paperi, non sarà meno interessante perché ha il valore di una lunga esperienza professionale, esercitata in ambiente scolastico nazionale di formazione di futuri cantanti, come quello dell’Accademia di Santa Cecilia in Roma.Giovanna Bruno
