Kenneth R. Silk , Peter Tyrer
editore: Cambridge
pagine: 576
This is a book of psychiatry at its most practical level. It answers the sorts of questions psychiatrists ask on a daily basis. What treatments are available for the condition this patient has? What is the evidence for the effectiveness of each of these treatments? Is there any value in combinations of treatment? And, can I be sure that the evidence and recommendations are free from bias? This handy pocket reference summarises the key elements of the Cambridge Textbook of Effective Treatments in Psychiatry. The text has been revised to provide the most up-to-date evidence for treatment decisions, with an accompanying summary table for each chapter to reinforce the recommendations. It is an essential guide for mental health professionals of all disciplines as well as medical students and trainees.

Handbook of Mentalizing in Mental Health Practice
Anthony W. Bateman , Peter Fonagy
editore: American Psychiatric Publishing
pagine: 624
Mentalization-based therapy is a specific type of psychotherapy designed to help people consider their own thoughts and feelings and differentiate them from the perspectives of others. The editors are the foremost experts on mentalizing, having published two previous books and a multitude of scholarly papers defining it and describing its multiple clinical applications. Handbook of Mentalizing in Mental Health Practice is by far the most cutting-edge, comprehensive source of information and instruction on this critical therapeutic technique, with everything clinicians need to know to integrate mentalizing into their therapeutic repertoire. The editors maintain that the aim of mentalizing therapy is to enhance a mentalizing process, regardless of the context in which it is being delivered. Thus, while most often employed in individual therapy, it can also be useful in group and family therapy situations. Similarly, it may prove equally effective in inpatient and outpatient contexts, and in standard and brief therapy modes.
What is critical is the therapist's focus on the patient's "mind-mindedness" as it applies to his or her subjective experience of reality and to awareness of other people's perspectives. Here are some of the key observations made in this fascinating book: * Evidence suggests that people who have been diagnosed with a personality disorder may have specific deficits in mentalizing in the context of attachment relationships, and that this group transcends the classification of borderline personality disorder (BPD), which the authors had previously identified with such deficits. This has profound implications for treatment of all types of personality disorders.* The authors now see mentalizing as a developmental construct that is extended to the family and throughout an individual's development. This insight suggests that intervention -- and even prevention of deficits -- may be possible.* Because mentalizing is a fundamental psychological process, it interfaces with all major mental disorders.
This means that mentalizing techniques may have the potential to improve well-being across a range of disorders, including depression, eating disorders, addiction, and even the less severe forms of antisocial personality disorder.* Adolescence, the phase of development where personality disorder, depression, eating disorders, substance abuse, and other disorders first emerge, is a critical period for identification of mentalizing deficits and the time when intervention can do enormous good. These insights are tremendously useful for any practitioner of psychotherapy, as well as students in the field. Exhaustive in its coverage of the nature, practice, and exciting potential of this relatively new approach, Handbook of Mentalizing in Mental Health Practice is destined to become a classic in the literature of psychotherapy.

Preventing Patient Suicide
Clinical Assessment and Management
Robert I. Simon
editore: American Psychiatric Publishing
pagine: 272
Today's psychiatrists practice in an environment that poses difficult challenges. Both treatment time and duration are limited by insurance requirements; many facilities are understaffed; split treatment arrangements are typical; and high-risk, acutely suicidal patients are admitted to inpatient units for short lengths of stay. In addition, law now plays a pervasive role in the practice of psychiatry. The doctor-patient relationship is no longer defined solely by the involved parties. Clinicians must juggle these requirements and limitations while providing the very best care to their patients, especially those at high risk. Preventing Patient Suicide: Clinical Assessment and Management provides the wisdom of Dr. Robert I. Simon's vast clinical experience, combined with the latest insights from the evidence-based psychiatric literature, to offer a cutting-edge survey of suicide prevention and management techniques. The author: * Addresses sudden improvement in high-risk suicidal patients, a phenomenon both common and perilous, with techniques for determining whether the improvement is real or feigned.*
Explores in depth the misuse of suicide risk assessment forms, with emphasis on their inherent limitations.* Examines the many entrenched myths and traditions about suicide, exposing them to the critical light of evidence-based medicine, including the concept of "imminent suicide risk" and the myth of "passive suicide ideation".* Discusses the continuum of chronic and acute high-risk suicidal patients, the fluidity with which one can become the other, and the difficulty in assessing these patients.* Explores how the law and psychiatry interact in frequently occurring clinical situations, and the importance of therapeutic risk management. In addition, the book contains a variety of features that illuminate the subject and enhance the reader's understanding, including: * Inclusion of illustrative case studies, combined with commentary on commonly occurring but complex clinical situations. * Key points at the end of each chapter that identify critical information. * A Suicide Risk Assessment Self-Test, a teaching instrument that consists of fifty questions designed to enhance clinician suicide risk assessment by incorporating evidence-based risk and protective factors. Dr.
Simon provides a nuanced, empathic, yet pragmatic perspective on identifying, assessing, and managing the suicidal patient while successfully navigating a complex legal and clinical environment that poses its own risks to the practitioner.
